We are delighted to invite you to Crewkerne Health Centre’s Open Day, taking place on Wednesday, 19th February 2025, from 2:00 pm to 5:00 pm at West One Surgery.
This event offers an excellent opportunity to connect with various teams who support the health and wellbeing of our community. Visitors will have the chance to meet:
- Our Nursing Team – Discover more about the care and services they provide, including chronic disease management, vaccinations, and health advice.
- Practice Administration and Management Team – Learn about the essential work that keeps the practice running smoothly and supports patient care.
- Primary Care Network (PCN) and Personalised Care Team – Find out about the collaborative services available through the PCN and how personalised care plans enhance patient outcomes.
- Patient Participation Group (PPG) – Engage with our PPG to learn how they represent patient voices and provide valuable feedback to improve services.
We are also pleased to showcase our Anima Help Desk, where you can receive personalised guidance on how to register for and use the Anima online system. This platform makes it easier to communicate with the practice and manage your healthcare needs.
